Our Model of Care: The Woven Blanket Model
At the Sweetgrass Health Centre (SGHC), we embrace the Woven Blanket Model of Care, a client-focused approach developed by Shkagamik-Kwe Health Centre (SKHC) that reflects the interconnectedness of individuals, families, and communities. This model honors the holistic traditions and values of Anishnawbek and other Indigenous peoples, ensuring culturally safe and comprehensive care for all.
The Woven Blanket Model empowers clients to actively determine who their helpers are in their care journey. These helpers may include SGHC team members, community services, friends, family, and traditional or Western healthcare providers.
Key features of the model include:
- Community Connections: SGHC provides a space for Indigenous individuals and families to connect with others in an urban context, fostering support networks and cultural belonging.
- Holistic Teamwork: Clients are supported by a team of providers who share responsibilities. While each client has a principal primary care provider (e.g., nurse practitioner, physician, or physician assistant), the team collaborates to ensure continuity of care. This means no client is left unsupported, even if their primary provider is unavailable.
- Integrated Programs: The Traditional Programs and the Four Directions Mental Wellness Program are integral to the model, combining cultural, emotional, physical, and spiritual care for clients and families.
- Client Navigators: Unique to the Woven Blanket Model, Client Navigators coordinate care, serve as points of communication, and advocate for clients within the healthcare system.
- Case Management Approach: Regular team meetings bring together Traditional, Mental Wellness, and Clinical Programs to discuss priority cases within a circle of care, enabling collaborative problem-solving and resource sharing.
At its heart, the Woven Blanket Model of Care fosters a culturally safe environment that respects each individual’s unique needs within the context of their family and community. It is a shared journey of healing and wellness, guided by the collective strength of SGHC’s interprofessional team and the wisdom of Indigenous traditions.
