Who We Are

The Sweetgrass Health Centre (SGHC) is a cornerstone of culturally centered healthcare, proudly serving as one of the province’s newest Indigenous Interprofessional Primary Care Teams (IIPCT). As a sacred branch of Shkagamik-Kwe Health Centre (SKHC), SGHC is dedicated to delivering holistic, high-quality care that seamlessly weaves the traditions, knowledge, and values of First Nation, Métis, and Inuit communities into every aspect of our services.

Sweetgrass Health Centre (SGHC) is an Aboriginal Health Access Centre (AHAC) dedicated to providing equal access to quality health care to First Nation, Métis and Inuit individuals and their families who live in the Township of Cochrane, or who reside onTaykwa Tagamou Nation, Constance Lake First Nation or Apitipi Anicinapek Nation.

 

 

We are a culturally based wholistic health centre dedicated to leading balanced and healthy lifestyles. Programs and services support traditional values encompassing the connectedness of emotional, spiritual, physical and mental well-being.
